What happens when you contact AgeWell Choices?
When you, your loved one, or your health care professional contact AgeWell Choices, you will receive a free telephone consultation with a trained social worker and an appointment for a visit to your home. During the home visit the social worker will meet with you and your family members to identify the problems that require services. The social worker will develop a plan tailored to meet your unique needs. Services are provided either on a short or long-term basis.
